ID & Insurance

Please bring with you your insurance card and driver’s license. Deductibles and co-payments are due and will be collected at the time of your visit. You will be responsible for payment at the time of service if you arrive without your insurance card. Your insurance is a contract between you and your carrier. Our staff will assist you to the best of their ability in assisting with your insurance company, but it is your responsibility to know your insurance policy and coverage of your plan before you arrive for your visit. If you are choosing to use your point of service or Out of Network Options, we recommend that you contact your insurance carrier prior to coming to our office to notify them that you are using this option for our team.
In order to serve you in a timely manner, we ask that you arrive 15- 20 minutes prior to your appointment time to fill out paperwork.

Surgery Day Check List

On the Day of Your Surgery

 Leave your valuables at home, including your watch, rings and other jewelry.

 Wear comfortable, loose fitting clothing that is easy to take off and put on.

 Shower or bathe the morning of surgery. Trim and clean your nails if you are having hand or foot surgery. Do not wear cosmetics if you are scheduled for facial surgery.

 Bring your health evaluation form and insurance identification.

 If you wear glasses or contact lenses, bring a case for safekeeping. You may wear your hearing aids and/or dentures. Denture containers will be provided if needed.

 Convenient, free parking is available near the A+M Surgical Center main entrance.

 You will be registered and asked to sign consent forms for treatment, surgery, anesthesia and insurance billing when you arrive.

 Do not eat or drink anything including water, coffee or tea.

 The anesthesiologist will review the nature of your surgery, your overall health and medical history and select the most appropriate anesthetic for you. General anesthesia enables you to 'sleep” through the procedure. Local anesthesia numbs only the area being operated on.

 The person accompanying you can remain with you until surgery. During surgery, he or she can wait in the surgical waiting area.
